ABSTRACT

Aim: To evaluate the resistance of the union between a glass fiber post and radicular dentine after cleaning the root with 17% EDTA and filling with different endodontic cements. Methods: Forty uniradicular bovine incisors were removed to obtain root lengths of 18 mm. Endodontic treatment was performed on all roots using different filling cements (zinc oxide and eugenol-based, OZE; cement based on epoxy resin, AH) and cleaning solutions (saline, SA or EDTA), which made it possible to obtain four groups: OZESA, OZEEDTA, AHSA and AHEDTA. Subsequently, 12 mm of filling material was removed from the roots, and they were prepared to receive fiber posts luted with resin cement. To execute the mechanical cycles (2x106 cycles, 90 N, 4 Hz), coronal reconstruction was performed with a silicon matrix. The roots were then sliced (2-mm thick) to perform the push-out test. The results were analyzed using analysis of variance (one factor and two factors) and Tukey's test (α=0,05). Results: Bond strength (Mpa) was significantly higher for OZEEDTA (9,18) and AHEDTA (8,70) than for OZESA (6,06) AHSA (8,7). OZEEDTA also presented the highest values in the cervical region (15,18) but was significantly lower in the apical region (2,99). However, AHEDTA had a homogeneous bond strength in all thirds. Conclusion: Regardless of the endodontic cement used, EDTA was used as an irrigating solution, culminating in a higher bond strength between the glass fiber post and dentin

ABSTRACT

OBJECTIVES@#This study aimed to observe the color rebound and rebound rates of non-pulp discolored teeth within 1 year after routine internal bleaching to guide clinical practice and prompt prognosis.@*METHODS@#In this work, the efficacy of bleaching was observed in 20 patients. The color of discolored teeth was measured by using a computerized colorimeter before bleaching; immediately after bleaching; and at the 1st, 3rd, 6th, 9th, and 12th months after bleaching. The L*, a*, and b* values of the color of cervical, mesial, and incisal parts of the teeth were obtained, and the color change amounts ΔE*, ΔL*, Δa*, and Δb* were calculated. The overall rebound rate (P*) and the color rebound velocity (V*) were also analyzed over time.@*RESULTS@#In 20 patients following treatment, the average ΔE* of tooth color change was 14.99. After bleaching, the neck and middle of the teeth ΔE* and ΔL* decreased in the 1st, 3rd, 6th, 9th, and 12th months, and the differences were statistically significant. Meanwhile, from the 9th month after bleaching, the rebound speed was lower than that in the 1st month, and the difference was statistically significant. The incisal end of the tooth ΔE* and ΔL* decreased in the 6th, 9th, and 12th months after bleaching, and the differences were statistically significant. No significant difference was found in the rebound speed between time points. However, this rate settled after the 9th month, with an average color rebound rate of 30.11% in 20 patients.@*CONCLUSIONS@#The results indicated that internal bleaching could cause a noticeable color change on pulpless teeth. The color rebound after bleaching was mainly caused by lightness (L*), which gradually decreased with time, and it was slightly related to a* and b*. The color of the teeth after internal bleaching rebounded to a certain extent with time, but the color rebound speed became stable from the 9th month. Clinically, secondary internal bleaching can be considered at this time according to whether the colors of the affected tooth and the adjacent tooth are coordinated and depending on the patient's needs.

ABSTRACT

Objetivos: Estudiar con tomografías computadas de haz cónico (CBCT) las lesiones perirradiculares de primeros molares superiores tratados endodónticamente; evaluar la lo- calización y, en los que tenían lesión en la raíz mesiovestibu- lar (MV), evaluar la frecuencia del conducto MV2. Materiales y métodos: A partir de una muestra de 179 tomografías, que incluían primeros molares superiores, se seleccionaron y analizaron aquellas en las cuales estos ha- bían recibido un tratamiento endodóntico previo, presentaban lesiones perirradiculares y conservaban una restauración co- ronaria. Las variables analizadas fueron: la localización de áreas radiolúcidas compatibles con lesiones perirradiculares de origen endodóntico y la correlación entre diferentes loca- lizaciones y la calidad del tratamiento realizado (adecuado, defectuoso o sin tratamiento). Los datos fueron analizados mediante la prueba de chi-cuadrado y la prueba de Spearman. Resultados: De las 179 CBCT que incluían primeros molares superiores, 90 reunían las condiciones requeridas. La frecuencia de las lesiones perirradiculares fue significativa- mente mayor en la raíz MV al compararla con las raíces disto- vestibular y palatina (p<0,05). El conducto MV1 presentó un tratamiento adecuado en todos los casos. El conducto MV2 recibió un tratamiento adecuado en 4 casos (4,44%), defec- tuoso en 4 casos (4,44%) y no tratado en 82 casos (91,1%); con una diferencia estadísticamente significativa (p<0,05). Conclusiones: En la muestra analizada, la localización de lesiones perirradiculares postratamiento endodóntico en primeros molares superiores se encontró con mayor frecuencia asociada a la raíz mesiovestibular, donde en la mayoría de los casos el con- ducto MB2 no fue tratado o presentó un tratamiento defectuoso (AU)


Aim: To study with cone-beam computed tomography (CBCT) the periradicular lesions of maxillary first molars with endodontical treatment; to evaluate its localization and, in those with a mesiobuccal root (MB) injury, assess the fre- quency of the MB2 root canal. Materials and methods: From a sample of 179 tomog- raphies (CBCT) that included maxillary first molars, those in which they had received previous endodontic treatment, pre- sented periradicular lesions and had a coronary restoration were selected and analyzed. The analyzed variables were: localization of radiolucent areas compatible with periradic- ular lesions of endodontic origin and the correlation between different localizations and quality of the performed treatment (adequate, defective, or untreated). Data were analyzed by the chi-square test and the Spearman test. Results: Of the 179 CBCT that included maxillary first molars, 90 met the required conditions. The frequency of periradicular lesions was significantly higher in the MB root when compared with distobuccal and palatal roots (P<0,05). The MB1 canal presented an adequate treatment in all cas- es. The MB2 canal received adequate treatment in 4 cases (4.44%), a defective one in 4 cases (4.44%) and was untreated in 82 cases (91.1%). The differences among these frequencies were statistically significant (P<0.05). Conclusions: In the analyzed sample, the localization of periradicular lesions after endodontic treatment in maxillary first molars was associated more frequently with the mesio- buccal root in which in the majority of cases the MB2 canal was untreated or presented a defective treatment

ABSTRACT

Las evaluaciones radiográficas de tratamientos endodón- ticos realizadas por graduados muestran un alto porcentaje de procedimientos incorrectos. Esta circunstancia lleva a la rea- lización de un elevado número de retratamientos ortógrados y retrógrados, con los inconvenientes y desventajas que conlle- va recurrir a una reintervención endodóntica. Es responsabili- dad de los profesionales, docentes y autoridades universitarias y gubernamentales revertir esta situación que afecta a la salud bucal de la sociedad. En el presente editorial se proponen di- ferentes alternativas para intentar modificar este preocupante panorama (AU)


Radiographic evaluations of endodontic treatments per- formed by graduates show a high percentage of incorrect procedures. This circumstance leads to the performance of a high number of orthograde and retrograde retreatments, with the inconveniences and disadvantages that entails resorting to an endodontic reintervention. It is the responsibility of pro- fessionals, teachers, university and government authorities to reverse this situation that affects the oral health of society. In this editorial, different alternatives are proposed to try to modify this worrying outlook (AU)

ABSTRACT

Objetivo: Evaluar radiográficamente tratamientos en- dodónticos del sector posterior de la cavidad bucal y com- parar la frecuencia de aceptabilidad en cada pieza dentaria involucrada. Materiales y métodos: Se evaluaron 5000 radiogra- fías de archivo de tratamientos endodónticos realizados apro- ximadamente entre 2005 y 2019 en premolares y molares mandibulares y maxilares en Argentina. Se consideraron tratamientos correctos e incorrectos de acuerdo con: 1) con- formación de la preparación quirúrgica; 2) límite apical de la obturación; 3) homogeneidad de la obturación. Se obtuvo la fre- cuencia absoluta y relativa de correctos e incorrectos. El aná- lisis entre frecuencias y pieza dentaria se realizó con la prueba de chi cuadrado y el cálculo del coeficiente V de Cramer. Para la comparación entre grupos de piezas dentarias se utilizó la partición del valor de chi cuadrado obtenido en los corres- pondientes grados de libertad. Nivel de significación P <0.05. Resultados: La partición del valor de chi cuadrado no mostró diferencias significativas entre primeros y segundos premolares mandibulares. Las otras comparaciones exhibie- ron diferencias significativas. Conclusiones: Un alto porcentaje de los tratamientos endodónticos de la población estudiada tiene por lo menos una condición que permite categorizarlos como incorrectos según el criterio establecido en este trabajo. Este porcentaje es más preponderante en anatomías complejas (AU)


Aim: To evaluate radiographically endodontic treat- ments performed in the posterior area of the oral cavity and compare the frequency of acceptability in each tooth involved. Materials and methods: 5,000 archival radiographs of endodontic treatments performed between 2005 and 2019 on mandibular and maxillary premolars and molars in Argentina were evaluated. The percentages of correct and incorrect treatments were considered according to 1) shaping of the preparation; 2) apical limit of the obturation; 3) homogeneity of the obturation. The absolute and relative frequencies of correct and incorrect treatments were calcu- lated. The association between these frequencies and tooth type was analyzed using the chi-square test and Cramer's V coefficient. For the comparison between groups of teeth, the partition of the chi-square value obtained in the corre- sponding degrees of freedom was used. Level of significance was P <0.05. Results: The partition of the chi-square value did not show a significant difference between the first and second lower premolars. The differences were significant in the other comparisons. Conclusions: A high percentage of the endodontic treat- ments in the study population have at least one condition war- ranting their classification as incorrect according to the crite- ria established in this study. This percentage is more prevalent in complex anatomies (AU)

ABSTRACT

O objetivo deste relato de caso foi descrever em detalhes a técnica de transfixação de pino de fibra de vidro no sentido horizontal e restauração de resina composta em um dente molar tratado endodonticamete. Paciente, sexo feminino, 51 anos de idade, buscou o Centro Universitário da Serra Gaúcha (FSG ­ Caxias do Sul ­ Rio Grande do Sul) com a necessidade de realizar tratamento endodôntico do elemento molar número 26. Após a avaliação inicial, foi realizada a endodontia. O elemento havia pouca sustentação das paredes vestibular e palatina, então foi proposto a colocação de pino intrarradicular seguido de coroa, entretanto, por questões financeiras a paciente não aceitou. Logo, foi proposto a técnica de transfixação horizontal de pino de fibra de vidro associado a uma restauração de resina composta de forma direta. O procedimento foi realizado em sessão única e foi utilizado um pino de fibra de vidro número 1 disposto transversalmente entre as paredes mesiopalatina e distovestibular. A restauração foi realizada com resina composta Filtek Z350, devolvendo estética e função ao elemento dentário. Pode-se concluir que a técnica de transfixação horizontal de pino de fibra de vidro associado à resina composta é uma alternativa restauradora que possibilita maior resistência aos dentes tratados endodonticamente, apresenta resultados funcionais e estéticos satisfatórios, além de possibilitar uma maior sobrevida aos mesmos(AU)


The purpose of this case report was to describe in detail the technique of horizontal transfixion of a fiberglass post into an endodontically treated tooth. Female patient, 51 years old, sought the University Center of Serra Gaúcha (FSG - Caxias do Sul ­ Rio Grande do Sul) with the need to perform the endodontic treatment of molar element number 26. After the initial evaluation, endodontics was performed. The element had little support for the buccal and palatal walls, so an intraradicular postplacement followed by a crown was proposed, for financial reasons the patient did not accept it. Therefore, the technique of horizontal transfixion of a fiberglass pin was proposed, associated with a restoration of direct composite resin. The procedure was performed in a single session and a number 1 fiberglass post was fixed crosswise between the mesiopalatal and distobuccal walls. The restoration was carried out with composite resin Filtek Z350, restoring aesthetics and function. It can be concluded that the horizontal transfixion of a fiberglass post associated with composite resin technique is a restorative alternative that allows greater resistance to endodontically treated teeth, provides satisfactory esthetics and better survival rates(AU)

ABSTRACT

Este trabalho teve como objetivo apresentar um relato de caso clínico de uma restauração do tipo coroa endocrown cerâmica em um dente molar tratado endodonticamente. Paciente com 45 anos de idade, sexo feminino, procurou a clínica do Centro Universitário da Serra Gaúcha (FSG) relatando a necessidade de reabilitação protética no primeiro molar inferior do lado esquerdo. Com base no exame radiográfico e exame clínico foi constatado um primeiro molar inferior do lado esquerdo com tratamento endodôntico satisfatório, mas uma extensa destruição coronária. Após a aprovação da paciente e indicações para a utilização da técnica, optou-se pela confecção de uma coroa endocrown em cerâmica reforçada por dissilicato de lítio. Foi realizado o preparo do remanescente dentário, envolvendo a câmara pulpar, com ângulos arredondados e expulsivos e, após a moldagem do mesmo com silicone de adição, utilizando a técnica simultânea e duplo fio. O provisório foi realizado com resina quimicamente ativada na cor 62 e, na sessão seguinte a coroa endocrown foi ajustada e polida. Após receber o protocolo de condicionamento adequado da peça, a mesma foi cimentada com cimento autopolimerizável Multilink N. Pode-se concluir que a alternativa restauradora Endocrown é um tratamento conservador e favorável para dentes com extensa destruição coronária, permitindo adequada estética e função(AU)


This study aimed to present a clinical case report of an endocrown ceramic crown restoration in na endodontically treated molar tooth. A 45-year-old female patient sought the clinic at the Centro Universitário da Serra Gaúcha (FSG), reporting the need for prosthetic rehabilitation on the lower left first molar. Based on the radiographic and clinical examination, the first molar presented satisfactory endodontic treatment, but extensive coronary destruction. After the approval of the patient and indications for the use of the technique, it was decided to make an endocrown with reinforced ceramic by lithium disilicate. The remaining tooth was prepared, involving the pulp chamber, with rounded and expulsive angles and, after the impression with silicone using the simultaneous and double wire technique was taken. The provisional was made with chemically activated resin in color 62 and, in the following session, the endocrown crown was adjusted and polished. After receiving the proper conditioning protocol, it was lutted with Multilink N self-curing cement. It can be concluded that the Endocrown restorative alternative is a conservative and favorable treatment for teeth with extensive coronary destruction, allowing adequate aesthetics and function(AU)

ABSTRACT

Este trabalho objetivou realizar duas revisões sistemáticas com as seguintes propostas: 1) Avaliar se a fototerapia com laser resultaria em menor dor pós-operatória (PP) em pacientes submetidos a reintervenção endodôntica; e 2) Avaliar se o uso da terapia fotodinâmica antimicrobiana (aPDT) seria eficaz na desinfecção de canais radiculares em casos de reintervenção endodôntica. As Revisões Sistemáticas foram registradas no PROSPERO (CRD42021243500 e CRD42021260013, respectivamente) e seguiram as diretrizes dos Itens de Relatório Preferenciais para Revisões Sistemáticas e Meta-análise (PRISMA). As buscas foram realizadas nas bases de dados eletrônicas PubMeb, Scopus, Web of Science, Embase, Web of Science, Clinical Trials e Cochrane Library e nos bancos de dados da literatura cinza. A qualidade metodológica e o risco de viés foram avaliados pela ferramenta Cochrane Risk of Bias para ensaios clínicos randomizados (RCT) e pelo qualificador NewcastleOttawa (NOS) para estudos não RCT (prospectivos). A análise da qualidade de evidência foi realizada com base na abordagem GRADE. A meta-análise foi realizada com o R software Meta package, utilizando um intervalo de confiança (IC) de 95%. Quanto aos resultados do Artigo 1: Cinco artigos foram incluídos para análise. Os estudos foram classificados como "baixo" risco de viés. Dos cinco estudos clínicos, quatro estudos mostraram uma diminuição significativa da PP após a reintervenção endodôntica nos grupos de fototerapia a laser quando comparados ao grupo controle, principalmente nos primeiros dias após a intervenção. A certeza de evidência foi classificada como baixa. Devido à alta heterogeneidade clínica entre os estudos, não foi possível realizar qualquer meta-análise. Apesar das limitações desta revisão sistemática, a fototerapia se mostrou uma alternativa promissora na redução e controle da PP na reintervenção endodôntica não cirúrgica. Nos resultados do Artigo 2, dez estudos atenderam aos critérios de elegibilidade e foram incluídos, sendo 8 utilizados na síntese quantitativa. A meta-análise mostrou que todos os dados dos estudos apresentaram diferença significativa antes e depois da terapia fotodinâmica antimicrobiana na redução da carga microbiana em infecções endondônticas secundárias (OR 0,15 [0,07; 0,32], p < 0,0001). No geral, os estudos apresentaram baixo risco de viés e a análise das evidências foi classificada como moderada. Sugere-se que a terapia fotodinâmica seja uma ferramenta benéfica e promissora, mostrando eficácia na redução da carga microbiana nos casos de reintervenção endodôntica. Em suma, a abordagem da utilização da fototerapia se demonstrou eficaz na diminuição da dor pós-operatória e na desinfecção dos canais radiculares, podendo ser uma terapia indicada nos casos de reintervenção endodôntica(AU)


This study aimed to carry out two systematic reviews with the following proposals: 1) Evaluate whether laser phototherapy would result in less postoperative pain (PP) in patients undergoing endodontic reintervention; and 2) Evaluate whether the use of antimicrobial photodynamic therapy (aPDT) would be effective in disinfection of root canals in cases of endodontic reintervention. Systematic Reviews were registered in PROSPERO (CRD42021243500 and CRD42021260013, respectively) and followed the guidelines of the Preferred Reporting Items for Systematic Reviews and Metaanalysis (PRISMA). Searches were performed in the electronic databases PubMeb, Scopus, Web of Science, Embase, Web of Science, Clinical Trials and Cochrane Library and in the gray literature databases. Methodological quality and risk of bias were assessed by the Cochrane Risk of Bias tool for randomized clinical trials (RCT) and by the Newcastle-Ottawa (NOS) qualifier for non-RCT (prospective) studies. The analysis of the quality of evidence was performed based on the GRADE approach. The meta-analysis was performed using the R Meta package software, using a 95% confidence interval (CI). As for the results of Article 1: Five articles were included for analysis. Studies were classified as "low" risk of bias. Of the five clinical studies, four studies showed a significant decrease in PP after endodontic reintervention in the laser phototherapy groups when compared to the control group, mainly in the first days after the intervention. The certainty of evidence was rated low. Due to the high clinical heterogeneity between studies, it was not possible to perform any meta-analysis. Despite the limitations of this systematic review, phototherapy proved to be a promising alternative for the reduction and control of PP in conventional endodontic reintervention. In the results of Article 2, ten studies met the eligibility criteria and were included, being 8 used in the quantitative synthesis. The meta-analysis showed that all data from the studies showed a significant difference before and after antimicrobial photodynamic therapy in reducing the microbial load in secondary endodontic infections (OR 0.15 [0.07; 0.32], p < 0.0001). Overall, the studies presented a low risk of bias and the analysis of evidence was rated as moderate. It is suggested that photodynamic therapy is a beneficial and promising tool, showing efficacy in reducing the microbial load in cases of endodontic reintervention. In summary, the approach to the use of phototherapy has been shown to be effective in reducing postoperative pain and disinfection of root canals, and may be an indicated therapy in cases of endodontic reintervention(AU)

ABSTRACT

Introdução: O cisto radicular é o cisto odontogênico mais comum, com uma prevalência aproximada de 60%. Esta lesão é geralmente observada por radiografias de rotina ou a partir de uma tumefação local, mas basicamente é assintomática, com crescimento lento e se encontra vinculada à um dente desvitalizado, possuindo predileção por indivíduos do sexo masculino com faixa etária se enquadrando entre a terceira e quarta décadas de vida. Seu tratamento pode ser cirúrgico ou não, variando de acordo com a dimensão e localização da lesão. Relato de caso: Paciente feminino, 66 anos, com presença de cisto radicular em região anterior de maxila, vinculada ao elemento dentário 22. A mesma foi submetida à enucleação cirúrgica associada à curetagem local para remoção e diagnóstico adequado da lesão, a partir da análise anatomopatológica do espécime. Considerações finais: Por ser uma patologia muito comum nos maxilares, é pertinente que o profissional conheça suas características essenciais para o correto diagnóstico, bem como os tratamentos mais adequados para cada paciente e que, apesar de somente a realização do tratamento endodôntico ser uma opção, a ausência da avaliação histológica da lesão restringe o correto diagnóstico desta patologia... Introduction: Radicular cysts are the most common odontogenic cyst, with a prevalence of approximately 60%. This lesion is usually observed by routine radiographs or presence of local swelling, but it is basically asymptomatic, with slow growth and it is associated with the root apex of a nonvital tooth, with a predilection for male individuals with ages ranging between the third and fourth decades of life. Its treatment can be surgical or not, varying according to the size and location of the lesion. Case report: A 66 year old female, with the presence of a radicular cyst in the anterior region of the maxilla, associated to the dental element 22 was evaluated. She was underwent surgical enucleation associated with local curettage for removal and proper diagnosis of the lesion, based on the anatomopathological analysis of the specimen. Final considerations: As it is a very common pathology in the jaws, it is pertinent that the professional knows its essential characteristics for the correct diagnosis, as well as the most appropriate treatments for each patient and that, although only endodontic treatment is an option, the absence of histological evaluation of the lesion restricts the correct diagnosis of this pathology... ABSTRACT

Objetivo: Describir el tratamiento endodóncico de cuatro casos clínicos de reabsorción radicular desarrollada durante o tras un tratamiento ortodóncico. Casos clínicos: Después de la preparación y la desinfección de los conductos radiculares, estos fueron obturados por un tiempo prudencial con una medicación intraconducto de hidróxido de calcio, que luego fue reemplazada por MTA. Luego de un período de entre uno y cuatro años, según el caso, los tratamientos fueron evaluados clínica y radiográficamente. Los dientes que debieron ser extraídos fueron analizados histológicamente. Según la evolución clínica y radiográfica de los casos presentados, el uso inicial de hidróxido de calcio y su posterior reemplazo por MTA parecería ser un protocolo adecuado para completar el proceso de reparación de los tejidos apicales y perirradiculares en casos de reabsorciones radiculares provocadas por fuerzas ortodóncicas excesivas. Las enfermedades sistémicas deben ser tenidas en cuenta para la elección de la terapéutica (AU)


Aim: To describe the endodontic treatment of four clinical cases of permanent teeth suffering root resorption during or after orthodontic treatment. Clinical cases: Four clinical cases of permanent teeth with root resorption were endodontically treated. After the root canals were prepared and disinfected, they were medicated with calcium hydroxide. After an appropriate observation period, the canals were finally filled with MTA. The treatment outcomes were clinically and radiographically assessed at an interval between one to four years according to the clinical case. The histological findings of teeth that required extraction was also described. Our clinical and radiographic observations suggest that a temporary filling with calcium hydroxide and further obturation with MTA can provide a favorable intracanal environment for apical and periradicular tissue reparation. The clinician needs a full medical history to be aware of the systemic diseases in patients with root resorption that will be of importance for the proper selection of treatment (AU)

ABSTRACT

As reabsorções radiculares são classificadas em interna e externa e um correto diagnóstico é fundamental para o sucesso no tratamento e é de suma importância que a resolução clínica seja feita precocemente para que se tenha um prognóstico clínico favorável. Reabsorção interna não é comum em dentes permanentes, e seu processo se caracteriza por um aumento do canal radicular de forma oval. O presente estudo teve como objetivos demonstrar e descrever através de um relato de prontuário o tratamento de reabsorção radicular interna de incisivos superiores permanentes. Através da seleção de uma paciente pela disciplina de Triagem da clínica odontológica do Centro Universitário da Serra Gaúcha, a qual apresentou alterações radiográficas compatíveis com reabsorção radicular interna inflamatória na região de dois incisivos superiores permanentes, com lesão periapical estabelecida em ambos, concluiu-se o tratamento para então gerar este estudo do tipo retrospectivo observacional. A reabsorção quando não tratada pode avançar e envolver estruturas além do tecido conjuntivo inflamado e a lesão pode avançar em direção apical. Após estabelecido o diagnóstico, iniciou-se os tratamentos endodônticos dos elementos utilizando hidróxido de cálcio como medicação intracanal, após as trocas de medicações as reabsorções foram controladas e então, as obturações dos canais foram realizadas, buscando um preenchimento adequado para o caso. A reabsorção radicular interna inflamatória causa danos irreversíveis, mas seu processo foi controlado através de terapia endodôntica. Este desempenho é considerado uma patologia, e pode comprometer qualquer extensão da raiz, é comum que os casos permaneçam assintomáticos, portanto, deve ser tratado o mais breve possível assim que descobertos para evitar sua progressão, e consequentemente danos maiores(AU)


Root resorption is classified as either internal or external, and a correct diagnosis is essential for successful treatment. Internal resorption is not common in permanent teeth, and its process is characterized by an enlarged oval root canal. The present study aims to demonstrate and describe, through a medical record, the treatment of internal root resorption of permanent upper incisors. A patient was selected from the Triage Course of the Dental Clinic (Centro Universitário da Serra Gaúcha) who presented radiographic changes associated with inflammatory internal root resorption in two permanente upper incisors, with a periapical lesion established in both and a treatment was then carried out in order to generate this retrospective observational study. Resorption when left untreated can progress and involve structures beyond the inflamed connective tissue and the lesion can advance in the apical direction. After the diagnosis was confirmed, endodontic treatments were initiated using calcium hydroxide as an intracanal medication. After changing the medication, the resorption was controlled and then the canal fillings were performed, seeking an adequate filling for the case. Inflammatory internal root resorption causes irreversible damage, but its process has been controlled through endodontic therapy. This performance is considered a pathology, and can compromise any extension of the root, it is common for the cases to remain asymptomatic, therefore, it should be treated as soon as it's discovered to prevent its progression, and consequently greater damage(AU)

Introducción: Las alternativas de tratamiento de órganos dentales con gran destrucción en su estructura varían, no sólo por el material de restauración, sino también por el valor económico y estético. Dentro de las alternativas existe: corona convencional, endocorona cuyo objetivo principal es la elaboración de una restauradora que evita la colocación de postes intraconducto y endocorona con ausencia de una pared axial (EPA) que se realiza cuando una pared, mesial, distal, vestibular o palatina está ausente. Objetivo: Verificar si la endocorona EPA se comporta de igual manera que las coronas convencionales y endocoronas al medir su resistencia ante fuerzas de tracción. Material y métodos: Treinta premolares fueron tratados endodóncicamente, 10 fueron preparados para recibir una corona convencional (grupo A), 10 para endocorona (grupo B) y 10 para endocorona EPA (grupo C). Se realizaron fuerzas de tracción para obtener el valor máximo en el cual las coronas fallaron. Se realizó una prueba ANOVA para comparar los resultados. Resultados: Al someter a los tres tipos de coronas a fuerzas de tracción los resultados obtenidos fueron: 3.04 ± 0.55 MPa para la corona, 7.08 ± 1.6 MPa para la endocorona y 6.17 ± 1.12 MPa para la endocorona EPA. Conclusiones: No existió diferencia significativa entre la endocorona (7.08 MPa) y la EPA (6.17 MPa), convirtiéndose en una alternativa de tratamiento con buen pronóstico en la práctica diaria (AU)


Introduction: The alternatives of treatment of tooth with excessive wear vary not only by the restoration material but also by the economic and aesthetic value. Among the alternatives there is: conventional crown, endocrown whose main objective is the elaboration of a restorative that avoids the placement of intraconducting posts and endocrown without one axial wall (EPA) that is done when a wall; mesial, distal, vestibular or palatal is absent. Objective: To verify if the (EPA) behaves in the same way as the conventional crown and endocrown when measuring its resistance to tensile strength. Material and methods: 30 premolars were treated endodontically, ten were prepared to receive a conventional crown, 10 for endocrown and 10 for EPA. Tensile strength were performed to obtain the maximum value at which the crowns failed, an ANOVA test was performed to compare the results. Results: When the three types of crowns were subjected to tensile strength, the results obtained were; 3.04 ± 0.55 MPa for the crown, 7.08 ± 1.6 MPa for the endocrown and 6.17 ± 1.12 MPa for the EPA endocrown. Conclusions: There was no significant difference between the endocrown (7.08 MPa) and EPA endocrown (6.17 MPa) becoming an alternative treatment with good prognosis in daily practice (AU)

ABSTRACT

Objetivo: Describir una experiencia de capacitación de alumnos de grado en la evaluación radiográfica de tratamientos endodónticos realizados ex vivo. Material y métodos: Participaron un docente y 13 alumnos, cada uno de los cuales evaluó radiográficamente 100 tratamientos endodónticos realizados ex vivo para determinar si estos eran correctos o incorrectos. Posteriormente, el mismo docente, en una clase teórica, presentó a los alumnos otras imágenes radiográficas a fin de calibrar qué debería considerarse correcto o incorrecto. Luego fueron proyectadas nuevamente las radiografías de los 100 casos, y los alumnos efectuaron una nueva valoración. Para cada alumno, se determinó la frecuencia de concordancia con el docente en los 100 casos, antes y después de la calibración. Se categorizó cada observación como sin cambio, positiva (precalibración sin concordancia y poscalibración con concordancia) y negativa (precalibración con concordancia y poscalibración sin concordancia). En cada caso se calcularon la frecuencia para cada categoría, la frecuencia de concordancia entre cada alumno y el docente antes y después de la calibración, y la diferencia entre frecuencias. Resultados: La frecuencia absoluta de casos con concordancia alumno/docente sin cambios fue entre 65 y 85; con cambio positivo, entre 14 y 29; y con cambio negativo, entre 1 y 8. La concordancia antes y después de la calibración resultó entre 37% y 79,2%. Conclusión: La calibración mejoró parcialmente la capacitación de los alumnos para la evaluación radiográfica de los tratamientos endodónticos (AU)


Aim: To describe one experience of calibration in the radiographic evaluation of 100 endodontic treatments performed ex vivo in undergraduate students. Material and methods: One professor and 13 undergraduate students participated in this study, who independently radiographically evaluated 100 ex vivo endodontic treatments and determined whether each case was correct or incorrect. Later, the same professor presented a theoretical class to the students with other radiographic images in order to calibrate the difference between correct and incorrect treatments. Then the radiographs of the same 100 cases were projected and the students made a new evaluation. The frequency of agreement with the teacher was determined for each student in the 100 cases before and after the calibration. Each observation was categorized as without change, with positive change (pre-calibration without agreement and post-calibration with agreement) and with negative change (pre-calibration with agreement and post-calibration without agreement). The frequency for each category was calculated for each student. In each of the cases, the frequency of students in which concordances with the teacher were observed before and after calibration, and the difference between both frequencies were calculated. Results: The absolute frequency of cases with agreement of the students/teacher without changes varied between 65 and 85, with a positive change between 14 and 29 and a negative change between 1 and 8. The concordance before and after calibration varied between 37.0% and 79.2%. Conclusion: Calibration partially improved the training of students in radiographic evaluation of endodontic treatments (AU)

Resumen Introducción: El oscurecimiento de un diente anterior interfiere negativamente en el aspecto de la sonrisa, y varias son las causas que pueden ser responsables por este oscurecimiento. Objetivo: Describir las técnicas de blanqueamiento mixto e inmediato a través del reporte de dos casos clínicos. Caso 1: Individuo de sexo masculino, con Síndrome de Treacher Collins, se quejó sobre alteración cromática del diente 33, verificada mediante examen clínico, radiográficamente presencia de tratamiento endodóntico satisfactorio. Por lo que fue planeado el blanqueamiento interno mediante técnica mixta. Caso 2: Individuo de sexo masculino, con Síndrome de Apert reportó cambio cromático en el diente 22, observado en el examen clínico, radiográficamente presentando tratamiento endodóntico insatisfactorio. Se realizó retratamiento endodóntico y a los 6 meses se realizó blanqueamiento interno mediante técnica inmediata. Conclusión: El blanqueamiento dental ejecutado con las técnicas mixtas e inmediatas, devuelve la armonía de la sonrisa, recuperando el color ideal y elevando la autoestima a los pacientes.


Resumo Introdução: O escurecimento de um dente anterior interfere negativamente na aparência do sorriso, e várias são as causas que podem ser responsáveis por esse escurecimento. Objetivo: Descrever as técnicas clareadoras mista e imediata através do relato de dois casos clínicos. Caso 1: Indivíduo do gênero masculino com Síndrome de Treacher Collins, queixou-se de alteração cromática no dente 33, constatado no exame clínico, radiograficamente apresentando tratamento endodôntico satisfatório. O clareamento interno foi planejado e realizado pela técnica mista. Caso 2: Indivíduo do gênero masculino, com Síndrome de Apert, relatou alteração cromática no dente 22, constatado ao exame clínico, apresentando tratamento endodôntico insatisfatório. Foi realizada a reintervenção endodôntica e após 6 meses, foi realizado clareamento interno pela técnica imediata. Conclusão: O uso das técnicas clareadoras mista e imediata, resulta na devolução da harmonia do sorriso, recuperando a coloração ideal e devolvendo a autoestima aos pacientes.


Abstract Introduction: The darkening of a single anterior tooth negatively affects the smile's appearance, and several factors may cause this darkening. Objective: To describe the mixed and immediate bleaching techniques by reporting two clinical cases. Case 1: A male individual with Treacher Collins Syndrome. He complained of chromatic alteration in tooth 33, which was verified on clinical examination. X-ray imaging showed satisfactory endodontic treatment. Internal bleaching was performed with the mixed technique. Case 2: A male individual with Apert Syndrome reported chromatic alteration in tooth 22, observed on clinical examination. X-ray imaging showed unsatisfactory endodontic treatment. Endodontic retreatment was performed. Six months later, internal whitening was performed immediately. Conclusion: The use of mixed and immediate whitening techniques restores the smile's harmony, the tooth's ideal color, and patients' self-esteem.

Introdução: A alteração de cor dentária consiste em um dos principais aspectos que corroboram à desarmonia do sorriso. Em situações de dentes não-vitais que apresentem escurecimento, o clareamento dental interno tem sido considerado uma opção conservadora de tratamento, podendo ser efetuado por meio de diferentes técnicas (mediata, imediata e mista). Em quaisquer técnicas, é necessária a confecção de um tampão cervical previamente à aplicação do agente clareador. Objetivo: Consiste em revisar a literatura a respeito do clareamento interno, das principais técnicas pelas quais ele pode ser realizado, sobre a importância do tampão cervical para a prevenção de danos ao dente e para a execução de um tratamento seguro e eficaz. Metodologia: Realizou-se uma busca nas bases de dados eletrônicos PubMed, ScienceDirect, LILACS e SciELO; analisando as publicações entre o período de 2010-2020. Resultados: A técnica mediata consiste em uma das mais utilizadas e indicadas; a imediata, na configuração termocatalítica, encontra-se em desuso, devido à contraindicação do uso do calor. A técnica mista, sendo a combinação das duas técnicas, pode potencializar o resultado clareador. Em todas elas, deve-se efetuar um tampão cervical para evitar o extravasamento do material clareador pelos túbulos dentinários, prevenindo a reabsorção cervical externa, que constitui o principal risco do clareamento interno. Conclusão: Baseado na literatura, todas as técnicas podem ser realizadas, desde que executadas de modo apropriado, seguindo as medidas como a da não aplicação de calor e a da execução do tampão, se adequando às necessidades do paciente a fim de escolher a técnica mais indicada.


Introduction: The change in tooth color is one of the main aspects that corroborate the disharmony of the smile. In situations of non-vital teeth that suffer from darkening, internal tooth whitening has been considered a conservative treatment option and can be carried out through different techniques (mediate, immediate, and mixed). In any of these techniques, it is necessary to make a cervical tampon before the application of the bleaching agent. Objective: To review the literature regarding internal whitening, the main techniques used in this process, the importance of the cervical plug for the prevention of damage to the tooth, and the execution of safe and effective treatment. Methodology: A search was performed in the electronic databases PubMed, ScienceDirect, LILACS, and SciELO, analyzing publications from 2010 to 2020. Results: The mediated technique consists of one of the most used and indicated; the immediate, in the thermocatalytic configuration, has fallen in disuse due to the contraindication of the use of heat. The mixed technique, which is the combination of the two others, can potentiate the whitening result. In all of them, a cervical plug is necessary to avoid the extravasation of the whitening material by the dentinal tubules, preventing external cervical resorption, which is the main risk of internal whitening. Conclusion: Literature suggests all three techniques can be performed if correctly executed, following measures such as the non-application of heat and the execution of the buffer, adapting to the needs of the patient to choose the most suitable technique.

Introdução: A dentística a cada dia demonstra grandes avanços nas técnicas e materiais, sendo eles aplicados de forma a preservar a estrutura dentária. Neste sentido, os pinos de fibra de vidro (PFVs) se destacam como uma alternativa de pinos intrarradiculares para a reabilitação de dentes endodonticamente tratados com perdas estruturais superiores a 50%. Objetivo: Realizar uma revisão de literatura sobre os pinos de fibra de vidro enfatizando seus aspectos gerais, propriedades e considerações biomecânicas. Materiais e métodos: Realizou-se uma revisão bibliográfica de estudos publicados nos últimos 21 anos (2000-2021), por meio de busca nas bases de dados: PubMED/MEDLINE, SciELO (Scientific Eletronic Library) e Google Acadêmico. Para a pesquisa, foram utilizados os seguintes descritores: Dente não Vital (Tooth, Nonvital), Pinos Dentários (Dental Pins) e Técnica para Retentor Intrarradicular (Post and Core Technique). Após criteriosa filtragem, foram selecionados 30 trabalhos para inclusão no estudo, além de 10 livros considerados relevan-tes para esta revisão. Resultado:Os PFVs demonstram excelentes propriedades estéticas, facilidade de execução da técnica e baixo custo, biocompatibilidade com tecidos dentais e perirradiculares, além de características biomecânicas vantajosas, o que resulta na trans-missão de menos tensão para a estrutura dentária, diminuindo a probabilidade de fratu-ras. Conclusão: As inúmeras vantagens e o excelente comportamento biomecânico desses pinos explicam seu destaque frente aos demais retentores intrarradiculares, sendo esses, quando bem indicados, a primeira opção para a reabilitação de dentes tratados endodonti-camente com extensas perdas coronárias.


Introduction: Every day, dentistry demonstrates great advances in techniques and materials applied to preserve the tooth structure. In this sense, fiberglass posts (FGPs) attract attention as an alternative to intraradicular posts for the rehabilitation of endodontically treated teeth with structural losses greater than 50%. Objective: Review the literature on aesthetic fiberglass posts, emphasizing their general aspects, properties, and biomechanical considerations. Material and Methods: A literature review of studies published in the last 21 years (2000-2021) through a search on the databases: PubMED / Medline, Scielo (Scientific Electronic Library), and Google Academic. The following descriptors were used: Tooth, Nonvital, Dental Pins, and Post and Core Technique. After careful filtering, 30 articles were selected for inclusion in the study, in addition to 10 books considered relevant to this review. Results: The FGPs demonstrate excellent aesthetic properties, eas-iness of execution and low cost, biocompatibility with dental and periradicular tissues, in addition to advantageous biomechanical characteristics, which result in the transmission of less stress to the tooth structure, reducing the probability of fractures. Conclusion: The numerous advantages and excellent biomechanical behavior of these pins explain their prominence concerning other intraradicular retainers, which, when properly indicated, are the first option for the rehabilitation of endodontically treated teeth with extensive coronary loss. ABSTRACT Objective: To examine the level of the accumulating success of the modern Resin-Based Endodontic Surgery (RES) and comparison with Endodontic Microsurgery (EMS) and finally offer a replacement at the predicted final results of EMS. Material and Methods: MEDLINE, PubMed, Cochrane Library, Embase, ISI, Google Scholar have been utilized as electronic databases for systematic literature until 2019. Therefore, Endnote X9, which can be provided in the market, has been applied to manage the electronic titles. Searches have been made with keywords "Endodontic Microsurgery OR EMS", "Resin-Based Endodontic Surgery OR RES", "Regenerative Endodontic Therapy", "Root-End Filling", "Root-End Surgery", "Periapical Surgery" and "Endodontics". Thus, this systematic review has been conducted concerningthe basic investigation of the PRISMA Statement-Preferred Reporting Items designed for the Meta-analyses and Systematic Reviews. Results: A total of RES =19 and EMS =31 with potential pertinent abstracts and topics were discovered in manual and electronic searches. Then, three articles for RES and four studies for EMS publications satisfied our inclusion criteria necessary for systematically reviewing the studies. The analysis showed the success rate for EMS as equal to 1.16 times the probability of the success rate for RES. Conclusion: Micro-surgical procedures superiorly achieved the predictable high success rate for the Root-end surgery compared to conventional methods.

Introdução: Os pinos de fibra de vidro (PFVs) apresentam-se como uma alternativa prática e econômica capaz de reabilitar dentes endodonticamente tratados com perdas estruturais superiores a 50%. Objetivo: Realizar uma revisão de literatura sobre o protocolo clínico acerca da utilização dos PFVs, bem como explanar as diferentes técnicas que po-dem ser utilizadas para implementá-los. Materiais e métodos: Realizou-se uma revisão bibliográfica de estudos publicados entre 2000 e 2021 por meio da busca nas bases de dados: PubMED/Medline, Scielo (Scientific Eletronic Library) e Google Acadêmico. Para a pesquisa, foram utilizados os seguintes descritores: Dente não Vital (Non-vital Tooth), Pinos Dentários (Dental Pins) e Técnica para Retentor Intrarradicular (Post and Core Technique). Após criteriosa filtragem, foram selecionados 24 trabalhos para inclusão no estudo, além de 10 livros considerados relevantes para esta revisão. Resultados: Durante o diagnóstico e planejamento, é necessário avaliar determinados fatores para que se obtenha sucesso clínico, como: quantidade de estrutura dentária remanescente, posição dentária e forças oclusais recebidas e necessidades restauradoras e estéticas exigidas pelo caso e pelo paciente. Diante das particularidades de cada caso, o clínico deve selecionar entre as técnicas a mais ideal: Técnica do PFV direto/ técnica convencional; Técnica do pino de fibra de vidro anatômico; ouTécnica direta com PFV associado a pinos acessórios. Conclusão: A técnica do PFV apresenta passos clínicos simplificados e sua correta realização garante sucesso no tratamento. Diante de situações em que o PFV não apresenta correta adaptação, o clínico deve abrir mão da utilização da técnica convencional e utilizar as técnicas de personalização do PFV.


Introduction: Fiberglass posts (FPs) are presented as a practical and economical alternative capable of rehabilitating endodontically treated teeth with 50% structural losses greater than 50%. Objective: To review the literature on the clinical protocol for the use of FPs, as well as explain the different techniques that can be used. Materials and methods: A literature review of studies published between 2000 and 2021 was carried out through a search on the Scielo (Scientific Electronic Library) and Google Scholar databases. The following descriptors were used for the research: Non-Vital Tooth, Dental Pins, and Technique for Intraradicular Retainer (Post and Core Technique). After careful screening, we selected 24 papers and 10 books, considered relevant for this review. Results: During planning, it is necessary to determine specific factors for clinical success, such as the amount of remaining tooth structure, the tooth position and the occlusal forces received, and restorative and esthetic needs required by the case and the patient. Given each case's particularities, the clinician must select among the most ideal techniques: the direct FP technique/convention-al technique; the Anatomical fiberglass post technique, and the direct technique with FP associated with accessory posts. Conclusion: The FP technique has simplified clinical steps and its correct performance is successful in the treatment. Faced with situations in which the FP does not present adequate adaptation, the clinician must not use the conventional technique, using techniques for personalizing the FP.

ABSTRACT

La restauración ideal en los dientes tratados endodónticamente (DTE) ha sido un tema controversial y ampliamente discutido en la literatura odontológica. El alto índice de fracasos en los dientes endodónticos se asocia principalmente a la falta de un adecuado sellado coronario, motivo por el cual la restauración post endodóntica cumple un papel fundamental. Los propios tejidos del DTE constituyen el refuerzo más importante de la pieza dentaria. La odontología considerada hoy en día como ideal es la mínimamente invasiva. A su vez la restauración de elección deberá ser aquella que respete mayor cantidad de tejido remanente a la hora de seleccionar la restauración coronaria. Se deberá considerar la cantidad y la calidad del remanente dentario sano, como los requisitos funcionales de cada pieza en particular. Si bien existen diversos tipos de restauraciones que podrían rehabilitar un DTE, en este trabajo sólo nos referiremos a las de inserción rígida, específicamente a las endo-onlays, también denominadas endocrowns (AU)

ABSTRACT

Objective: The purpose of this study was to assess patient satisfaction, gross fracture and marginal adaptation of e.max press endocrowns versus e.max press crowns retained with Fiber reinforced composite post (FRCP) and core in upper anterior teeth. Material and methods: The present study included 24 patients seeking root canal treatment in anterior upper arch. The patients received root canal treatment (RCT) then they were randomly assigned into two groups (n=12). The first group received preparation for the IPS e.max crowns retained with FRCP and core and the second group received preparation for the IPS e.max endocrowns. Press technique was used for the fabrication of both restorations using IPS e.max press ingots. Marginal integrity and gross fracture were evaluated using USPHS criteria and a questionnaire was conducted to evaluate patient satisfaction. Data were analyzed using IBM SPSS Statistics for Windows, Version 23.0. Armonk, NY: IBM Corp. Results: There was no statistical significant difference regarding gross fracture of both groups after 12 months (p-value = 0.093, Effect size = 0.447), meanwhile; group 1 was statistically significantly higher than group 2 regarding marginal integrity (p-value = 0.037, Effect size = 0.513). Regarding patient satisfaction FRCP and core group showed statistical significant higher satisfaction than endocrown group (p-value = 0.047, Effect size = 0.447). Conclusion: E.max press endocrowns revealed successful performance similar to e.max press crowns retained with FRCP in terms of gross fracture, however better marginal adaptation and patient satisfaction was obtained with e.max press crowns retained with FRC post and core group. Objetivo: O objetivo deste estudo foi avaliar a satisfação do paciente, grau de fratura grosseira e adaptação marginal de endocrowns e.max versus coroas de e.max retidas com pino de compósito reforçado com fibra (FRCP) e núcleo nos dentes anteriores superiores. Material e métodos: O presente estudo incluiu 24 pacientes que buscavam tratamento endodôntico na arcada superior anterior. Os pacientes receberam tratamento de canal radicular (RCT) e foram divididos aleatoriamente em dois grupos (n = 12). O primeiro grupo recebeu preparação para as coroas IPS e.max retidas com FRCP e núcleo e o segundo grupo recebeu preparos para as endocrowns IPS e.max. A técnica de prensagem foi usada para a fabricação de ambas as restaurações usando os lingotes de prensagem IPS e.max. A integridade marginal e a fratura macroscópica foram avaliadas usando os critérios da USPHS e um questionário foi realizado para avaliar a satisfação do paciente. Os dados foram analisados usando IBM SPSS Statistics for Windows, versão 23.0. Armonk, NY: IBM Corp. Resultados: Não houve diferença estatisticamente significativa em relação à fratura bruta de ambos os grupos após 12 meses (p-valor = 0,093, tamanho do efeito = 0,447), entretanto; o grupo 1 foi estatisticamente significativamente maior do que o grupo 2 em relação à integridade marginal (p-valor = 0,037, tamanho do efeito = 0,513). Em relação à satisfação do paciente, o FRCP e o grupo principal mostraram maior satisfação estatisticamente significativa do que o grupo endocrown (p-valor = 0,047, tamanho do efeito = 0,447). Conclusão: as endocrowns E.max press revelaram um desempenho bem-sucedido semelhante às coroas e.max press retidas com FRCP em termos de fratura bruta, no entanto, melhor adaptação marginal e satisfação do paciente foram obtidas com as coroas e.max press retidas com pilar FRC e núcleo de preenchimento (AU)
